DHAR: The Danish Hip Arthroscopy Register - The First 2000 Patients

Summary: Results from the Danish National Hip Arthroscopy Register, surgical data and 1-2 year PROM data

Abstract:

Since February 1st 2012 the Danish Hip Arthroscopy Register has been open to Hip
surgeons to fill per-operative data. The surgeon registers multiple data and among
these are OR- and traction time, antibiotics, DVT prophylaxis, CE-angle, Alfa angle,
JSW and prominent Ischial spine. Surgery details regarding labral and cartilage surgery
are recorded and CAM and Pincer surgery is registered. Extraarticular procedures and
complications during surgery are also noted. Patients are asked preoperatively to fill out
on-line various Patients Related outcome Measures (PROM). These include VAS-rest,
VAS-15 min. Walk, iHOT12, HAGOS. All patients receive the same PROMS at 1, 2 and
5 years.
The DHAR database allows for easy data retrieval and all data can be exported to a
CSV-file. This makes it easy to work with data in spreadsheets or statistical
programmes.

Results

Since February 2012 2000 operations and 732 1 year and 251 2-year PROMS have been registered in DHAR.
57 % males and 43 % females were operated. Mean OR time was 89 minutes and
mean traction time was 49 minutes. Antibiotics was given to 99 % and DVT prophylaxis
to 28 % of the patients. Mean CE-angle was 32, Alpha angle 69 and AI-angle 7.
88 % of patients had a labral tear and of these 83 % had the labrum reattached with
mean 3 anchors.
During the procedure cartilage damage was found in 87 % of the hips and mainly on
the acetabular side. Grade 3 changes were found in 32 % of the cases and grad IV
changes in 11 % of the cases. In 170 patients a psoas tenotomy was performed, Mainly
at the level of the acetabular rim. The surgeons reported 238 peroperative complications
and the majority of these were suture breakage and anchor pull-outs during labral fixation.
Accidental cutting of the labrum was seen in 26 patients and 16 instruments broke inside
the hip joint.
PROM data 1 and 2 year results HAGOS and iHOT12: All subscales show significant
improvements with P-values < 0.05. VAS scores also show significant improvements at
1 and 2 years. More details with subscales will be presented
