here is a strong association and biomechanics mechanism between degeneration of anterior cruciate ligament with knee Posterior horn and root tear of the medial meniscus and conjunction with notchplasty. And the high incidence of root tear of the medial meniscus in knee with osteophyte of the ACL tibial check point.
Abstract
Objective
To explore the association of degeneration of anterior cruciate ligament (ACL) with conjunction with Intercondylar notch impingemenin in KOA(Knee osteoarthritis,KOA)and the medial meniscus tear.
Methods
Between July 2014 and February2016, 55 patients(55 knees) were diagnosed with degeneration of ACL and 55 random and normal ACL patients(55 knees, normal group) for control group were included in the study. Two groups of patient showed no statistically significant difference in gender, age, BMI, and side(P>0.05). MRI was examined for the evaluation of Notch width index in preoperative; The location of the medial and lateral meniscus tear and osteophyte of the ACL tibial insertions were observed under arthroscopy . Calculated the incidence of the meniscus tear and osteophyte.
Result: Two groups of patient showed no statistically significant difference in anterior horn and body tear of the medial meniscus and anterior horn, body,posterior horn, root tear of lateral meniscus(P>0.05).While , Significant difference could be detected in the analysis of Posterior horn , root tear of the medial meniscus ,osteophyte of the ACL tibial insertions, narrowed of intercondylar notch and the notch width index(P<0.05). The incidence of root tear of the medial meniscus in 13 knees with osteophyte of the ACL tibial insertion was 53.8%(7/13), and the incidence of root tear of the medial meniscus without osteophyte of the ACL tibial insertion was 16.5%(16/97). Significant difference could be detected (X²=9.671,P=0.002).
Conclusion
There is a strong association between degeneration of ACL with Posterior horn and root tear of the medial meniscus and conjunction with notch impingement in KOA. And there are high incidence of root tear of the medial meniscus in knee along with osteophyte of the ACL tibial insertions.
