Abstract
Background
Achilles tendon rupture is a serious injury in athletes. Due to high rate of re rupture, conservative trteatment has no role in this group of patients. Surgical repair of tendon is apply in Because of high rate of re rupture and other complications in conservative method surgical repair of acute Achilles tendon ruptures is considered superior to nonoperative treatment in most of these patients.
Purpose
In this study we evaluate the result and outcomes of this method of repair which has some advantages which incease the result in athletes. We believe that if the paratenon remain intact during repair, the healing process of the Achilles tendon and its gliding are guaranteed which help to effective early rehabilitation.
Methods
In this study after skin incision in Achilles tendon medial border, undermind the skin over the paratenon then repair the achill4es tendon with 2 sutures in modified Kessler method over the paratenon. Intact paratenon maintain the vascularity around the tendon and also provide early good gliding of the tendon in it, so healing in shorter time without any adhesion, help the tendon and calf muscles rehabilitate faster. Mean follow-up time was 36 months and all of them was assessed with ARTS scoring systems (Achilles Tendon Total Rupture Score).
Results
Our results show good to excellent functional outcome in ARTS (mean=79.2).In addition to good results, the patient become satisfied from short time post operation casting and rehabilitation program. There was no case of major complication such as any wound problems, nerve injury or re-rupture which are seen in non-operative treatment, open surgery or percutaneous procedures.
Conclusion
It seems this method of surgery for acute Achilles tendon rupture have acceptable results.
Other advantages of this method is short surgery time, run easily by every surgeon and low cost for patients.
