Abstract
Objective
The role of PRP has been investigated in both surgical and conservative treatent of rotaotr cuff
Purpose
of this study is to investigate the efficay of PRP in connservative treament of symptomatica partial supaspinatus tear
Material and methos
44 patients presenting symptomatic partial intrasubtanttial tear of the supraspinatus ( cm) less than 1.0 were included in this study
All the patients underwent the same rehabilitation program including assisted physical therapy treatment Group A 21 patiens underwent a 3 consecutive ultrasound guided PRP ijections with ACP system, in group B 23 patients had no PRP treatment as a control gropu. Active sports participation or heavy work was restricted in both groups for a 6 months period
The Shoulder Pain and Disability Index, passive range of motion of the shoulder, a physician global rating scale at 3 and 6 six-month follow-up, monitoring possible adverse effects treatment related .MRI control exam was achieved at fopllow-upo only in the PRP group
Results
The clinical effect of the platelet-rich plasma injection was superior to conservative treatment alone at 6 months period six months after initial injection (P < 0.05). At six months the mean Shoulder Pain and Disability Index was an average value of 18.9 in the platelet-rich plasma group versus 27.8 in no PRP treatment group (P < 0.05). No adverse effects were observed in either PRP and control group.MRI showed emprovement of the tissue signal or reduction of the lesion gap in 19/21 patients
Conclusions
Additional Autologous platelet-rich plasma injections in conservative treatment of partial supraspinatus lead to a achieve better outcomes in term of reduction in the pain and disability when compared to conservative treatment alone with effectivess mantained at 6 months follow-up
