Cemented VS Cementless CAS TKA: Mid-Term Reults of a Prospective Randomized Study

This study demonstrates that the superior accuracy in implant positioning of CAS TKAs in combination with a particular design of the implant (ultra-congruent mobile-bearing TKA) and the technology of HAp coating, permits to successfully avoid the cementation of components in TKAs.

Abstract
The optimal mode of fixation in total knee arthroplasty (TKA) is still a subject of debate. The majority of surgeons prefer cemented fixation. There have been reports in literature that showed that both clinical outcomes and long-term survival are inferior for cementless components, especially on the tibial side. Our study shows the results of a series of 243 consecutive CAS TKAs, 123 cemented and 120 cementless Score CAS TKAs implanted with the specific computer navigation system Amplivision (Amplitude, Valence, France). The study has been designed as a prospective randomized study and we present the results at 8 years of follow up. A statistical analysis has been performed by an indipendent institute. No patient was lost at follow up. There was one case of superficial infection (sustained by Staph. Aureus), successfully treated with antibiotics. The results show no statistically significant difference between the two groups for what regards satisfaction of patients, clinical outcome and radiographic assessment.
