ISAKOS Knee Arthroplasty Workshop Registration

To ensure that course participants have the appropriate skills, experiences, interests, and accessibility of
instrumentation within their region, individuals interested in attending the ISAKOS Knee Arthroplasty Surgical
Skills Workshop will be asked to first complete an application questionnaire. The questionnaire will ensure
that admitted participants are able to experience the best level of surgical skills training.
Completed applications will be reviewed by the Workshop Program Committee. If approved for admission,
applicants will be invited to register through a personal link that will be sent by the ISAKOS Office.
Individuals will then have one week to submit their registration, or their spot may be given to the
next applicant on the wait list.
The workshop is limited to 24 participants to maximize faculty-to-attendee interaction and hands-on training opportunities.

Registrants can pay for their registration fees in three ways: credit card, check, or wire transfer.

Credit Card - ISAKOS accepts American Express, Discover Card, MasterCard and Visa. Please note that charges will be processed immediately
when the registration is completed. If your credit card is declined, please try another card or contact the ISAKOS Office about alternate forms of payment.

Check - ISAKOS will accept checks for payment of registration fees. The check must be from a US banks in US dollars. The checks must be
mailed to the ISAKOS Office (please do not fax a copy of the check). Please note that your registration will not be completed if the check cannot be deposited.

Wire Transfer - Wire transfers are only available by contacting the ISAKOS. Wire transfers must be accompanied by a Wire Transfer
Authorization form and contain specific information related to the registrant. Wire transfer forms must be faxed to the ISAKOS Office with the registration
forms. Please note that registrations will not be processed until the wire transfer is received in the ISAKOS bank account.
Preferred pricing for the ISAKOS/S+N Workshop is available with ISAKOS Membership.

Cancellations
Cancellations will be subject to a US $50 processing fee and may take up to three weeks to be processed. Requests for refunds must be received in
writing in the ISAKOS Office by September 1, 2024. No refunds will be issued after September 1, 2024. Refunds will not be issued for any events
or services included in the Workshop that are not used at the discretion of the participant.
