The Obsolete Posterior Dislocation of Shoulder Treated With Modified Maclaughlin Surgery

Abstract

Purpose

To investigate the clinical outcomes after the modified MacLaughlin surgery in the treatment of obsolete posterior dislocation of shoulder joint.

Methods

From January 2008 to December 2011, 12 patients with posterior dislocation of shoulder which is undiagnosed in other hospital underwent the modified MacLaughlin surgery. Preoperational 3D-CT scanning shows the humeral head defect is less than 30%. During the surgery, we take the anteromedial approach of the shoulder joint. Separate through the deltoid and pectoralis major muscle, then exposure the subscapularis and the lesser tubercle. Cut off the tubercle of the humerus along with the subscapularis tendon. The bone block size is about 1.5cm * 1.5cm * 1.0cm. Retract the bone block to the interior side and try to reduce the lesser tubercle manipulatively, open the anterior capsule if it is necessary. Then put the lesser tubercle in front of the bone defect on humeral head. After fixed with 2-3 cannulated screws, set a negative pressure drainage tube then close the incision. The patients were evaluated preoperatively, 6 weeks and 3, 6, and 12 months postoperatively, and every 6 months thereafter. The clinical examination was undertaken using the Constant function score and correction scores.

Results

All patients were satisfied with the clinical results, same as the recovery of the shoulder joint function. The shoulder joint function Constant scores improved from 62.5 points (48 to 74) to 93.8 points (86 to 98) at the last follow-up visit.
Conclusions:The modified MacLaughlin surgery is a definitely effective method to treat the obsolete posterior dislocation of the shoulder joint, also is worth recommending.
