Abstract

Introduction

Allografts have been shown as a viable alternative to autograft in anterior cruciate ligament reconstruction (ACLR). But recent study has reported that soft tissue allograft failed at a rate over 3 times higher than HA autografts at a minimum of ten years after single bundle ACLR. The purpose of our study was to investigate the graft maturation after single bundle ACLR using hamstring (HA) autograft and tibialis anterior (TA) allograft on second-look arthroscopy and to compare the graft maturation between HA autograft and TA allograft.

Materials And Methods

We retrospectively reviewed 58 patients who underwent second-look arthroscopy after anatomic ACLR using HA autograft or TA allograft between 2007 and 2016. Graft maturation at second-look arthroscopy was evaluated with four items, including graft integrity (tear), synovial coverage, graft tension and graft vascularization. Maximum two points were assigned to each item depending on the status of reconstructed graft. Graft maturation score was summed from the score of four items. According to graft maturation score, graft maturation was rated as excellent (7-8), good (5-6), fair (3-4) and poor (0-2). Total graft maturation score and each item score were compared between two groups.

Results

HA group had better result in all item of measurement: graft integrity (1.8±0.5 vs 1.4±0.6, p=0.003), graft synovialization (1.4±0.7 vs 1.1±0.6, p=0.057), graft tension (1.9±0.5 vs 1.5±0.6, p=0.006), graft vascularization (1.3±0.8 vs 0.6±0.8 p=0.002). The difference of graft integrity, tension and vascularization showed statistical significance. Graft maturation score of HA group (6.3±1.9) was higher than TA group (4.5±1.9). There were statistical significance of graft maturation scores between two groups (p=0.000).

Conclusions

This study showed favorable graft maturation after anatomic single bundle ACLR using HA allograft at second-look arthroscopy compared to TA autograft.
