Abstract

Purpose

The purpose of the present study was to evaluate the outcomes of the minimally invasive double adjustable button fixation technique for ACL reconstruction in young patients. It was hypothesized that all patients will present improved outcomes over time.

Methods

This prospective study included thirty-on patients (20 males and 11 females) with mean age of 28.6 years (SD±2.8) who were treated for isolated ACL deficiency providing knee instability. All patients were treated by the same senior knee surgeon. A minimally invasive technique was performed using a quadruple semitendinosus graft and an adjustable suspensory button fixation both in femur and tibia. Primary outcome was knee anteroposterior translation as measured by the Lachman and Anterior Drawer tests. Secondary outcomes included patient reported outcome measures (PROMs), an assessment of health status and health-related quality of life provided directly from the patients.

Results

Complete data were recorded for all patients with a minimum of 2 years follow-up. All patients presented a severely abnormal anteroposterior translation (>10mm) pre-operatively whilst after reconstruction both a negative Lachman (<2mm) test with a firm anterior endpoint and a negative anterior drawer test were observed. IKDC subjective knee score was increased from 42 to 93 at the final follow-up. Mean Tegner Activity Scale was also improved from a mean of 2.1 to 5.9 at two years postoperatively. The KOOS evaluation showed increased scores in all subcategories. More precisely, KOOS-pain was improved from 58 (range 39.0-71.1) to 93.1 (range 89.0-100.0), KOOS-Symptoms from 61.9 (range 43.3-71.0) to 94.0 (range 88.5-96.1), KOOS-ADL 58.0 (range 45.8-75.6) to 91.0 (range 86.8-99.0) KOOS-Sports/Rec from 33.5 (range 20.1-45.7) to 630.0 (range 51.0-75.0) and KOOS-QOL from 34.6 (range 23.5-51.5) to 74.5 (range 68.5-85.0).

Conclusions

A double adjustable button fixation for ACL reconstruction is an efficient and safe procedure to treat knee instability with significant improvement in the objective and subjective scores in the demanding population of young patients.
