Abstract
Objective
To evaluate and compare the knee function and stability of patients undergoing medial collateral ligament reconstruction associated with the distal advancement technique of the direct arm of the semimembranosus in concomitance with the reconstruction of one of the cruciate ligaments.
Methods:A cohort study, based on the evaluation of patients diagnosed with chronic medial collateral ligament grade 3 injury associated with injury to one of the cruciate ligaments of the knee submitted to surgery. Twenty-five medical records were selected after applying the inclusion and exclusion criteria. They were submitted to the surgical technique of anatomical reconstruction of one of the cruciate ligaments and of the superficial medial collateral ligament with the gracilis muscle and semitendinosus tendons associated with the advancement of the semimembranosus through its detachment from the tibia and anterodistalization of the same. The Lysholm score and the valgus stress radiographs were calculated and analysed before and at 6, 12 and 24 months postoperatively.
Results
There was an increase (p< 0.05) in the Lysholm score, as well as a decrease (p<0.05) of the medial opening in the radiography with stress, comparing the periods in the two years of follow-up. There were no significant differences in the variables analysed in the comparison between groups.
Conclusion
The subjective and radiographic results obtained suggest that the technique of anterodistalization of the semimembranosus, used in this study, when associated with reconstruction of the medial collateral ligament and one of the cruciate ligaments, becomes a good option in knee dislocation type 1 cases with clinical signs of anteromedial rotatory instability.
