Abstract

Introduction

The present study points to the questioning of the efficiency of conventional magnetic resonance imaging (MRI) in our country for the evaluation of anterolateral ligament (ALL) in patients with anterior cruciate ligament (ACL) lesion. The hypothesis is that ALL injuries are not being properly visualized and are not being reported by radiologists.
Objectives: To evaluate the citation of the ALL in the MRI reports of 103 patients who underwent ACL reconstruction and confirm its presence and lesion in the images of exams performed in the acute phase in a retrospective way.

Methodology

Reports and images of 103 patients who underwent ACL reconstruction in 2019 were included. The images were reanalyzed by two radiologists confirming the presence and signs of injury to the ALL.

Results

In the first analysis, only one report mentioned the ALL and its injury (0.97%). On reanalysis, ALL was visualized in almost all cases (95% and 97%). A lesion was found in 53 (51.5%) cases by radiologist A and in 56 (54.4%) cases by radiologist B. The lesion was diagnosed by both in 39 (37.9%) cases (p<0.0001). There was disagreement among radiologists regarding the lesion (Kappa 0,411).

Conclusion

The reports failed to describe the ligament and diagnose a significant number of injuries. The analysis of conventional resonance images still generates divergences in the diagnosis of ALL injury associated with the ACL among radiologists.
