Abstract
Medial patellofemoral ligament reconstruction is a common procedure and one of the treatments of choice for patellofemoral instability, although there is no surgical consensus regarding graft choice, surgical technique and methods of fixation. Different techniques are described, most of them using different kind of implants, anchors, interference screws with an increase of stiffness after surgery and elevated cost of the implants. In some developing countries the cost of implants makes surgery impossible. We present a MPFL reconstruction technique free of implants, using autograft with a fithian´s technique at the patellar attach and a loop around the adductor tendon (adductor sling) for the femoral fixation. We report 32 cases, 26 isolated MPFL and 6 combined procedures, BPII 78 (0-100), 3 failures reported. This is a reproducible, effective, and NO COST surgery for patellofemoral instability. Socioeconomic and demographic factors should be considered for the selection of the surgical technique.
