Abstract
Purpose
This study aims to evaluate the clinical outcomes at a minimum 10-year follow up in a series of patients with post-meniscectomy syndrome treated with a polyurethane meniscal scaffold.
Methods
This is a prospective series that included 27 patients who underwent arthroscopic implantation of a polyurethane scaffold from 2008 to 2011. Clinical and radiological outcomes were assessed at preoperative period, at 5-years and at minimum 10-years of follow- up. Clinical outcomes were based on patient-reported outcomes; KOOS, IKDC, Lysholm and Tegner. Imaging evaluation was also assessed by radiographical techniques assessing the joint-space narrowing in the Rosenberg view. Failure was defined as patients with needed a surgery for removing the scaffold or those patients who need a surgery for a total or partial knee replacement.
Results
Twenty-one patients, with a mean age of 56 ± 9.8 year, were finally available for the final follow-up. Mean follow-up was 142.2 (range 123-153) months. All functional scores showed a significant improvement (p<0.001) at 5- and minimum 10- years follow-up with the exception of Tegner score which remained stable. The join-space width maintained from the preoperative period (1.9 ± 1.2 mm) up to the 5-years follow-up (1.32 ± 1.57 mm, n.s.) and decreased by the last evaluation (0.7 ± 1.12 mm, p=0.001) at the last follow-up.
Two patients (9.5%) out of 21 patients were converted to a total knee replacement during the study period.
Conclusions
The polyurethane meniscal scaffold in patients provides significant and stable pain relief overtime and good functional outcomes at minimum 10-years after surgery. However, degenerative changes were observed in the compartment treated during this period time.
