Summary: Remplissage procedure with single anchor yields excellent Filling of the defect and clinical outcome


Introduction

Remplissage in French means filling , is a technique in which an intra articular hill sach defect is converted into extra articular one by filling it with infraspinatus tendon during arthroscopic bankart repair procedure. Different techniques are used for it. While most of these techniques employ the usage of two suture anchors , there are lacunae in literature whether only one anchor is sufficient or not

Methods

The Study included 30 patients treated between 2015 and 2019 with anterior shoulder instability who underwent arthroscopic Bankart repair combined with remplissage using one suture anchor using double mattress technique. Patients in the study were analysed pre operatively and at 6 months follow up with CT and MRI . Clinically Rowe scores , Oxford Instability scores and Visual Analog Score( VAS ) scores were analysed . Radiologically hill sach index , Filling Index of Remplissage (FISOR) were analysed.
Results-At an average of 2 years of follow up , all patients had a good outcome with mean Rowe score 43 , Oxford score 95 and VAS score 1.9 . 21(70%) Patients on follow up had FISOR grade 4. None of the patients had post operative complications or recurrent instability at final follow up .

Conclusion

-In the remplissage procedure, the double mattress technique using a single double loaded anchor provided significant improvement in functional scores as well as good radiological outcome . This technique is a cost effective and good alternative in indicated cases of remplissage.
