Summary: The study presents a new arthroscopic technique for chronic lateral ankle instability (CLAI) using knotless anchors, demonstrates excellent short-term functional outcomes, and compares favorably to existing literature.


Introduction

Chronic lateral ankle instability (CLAI) is a common pathology in sports. The gold standard surgical treatment is the open surgery – modified Broström-Gould. In recent years, there has been a growing interest in using arthroscopy for the treatment of CLAI, however there are still few studies in this area.
Objectives: Introduce a new arthroscopic CLAI repair technique, comparing its outcomes to existing literature. The secondary objective was to compare the postoperative improvement in clinical and functional scores.

Methods

Data was collected from patients who underwent arthroscopic repair with a knotless anchors system at three different times: preoperative, 6 and 12 months postoperative. Surgical technique was described and variables such as age, sex, Visual Analog Scale (VAS), American Orthopaedics Foot and Ankle Society Score (AOFAS) Ankle-Hindfoot Scale, Karlsson-Peterson score, patient satisfaction, and surgical-related complications were collected.

Results

36 participants were included (64% male and 36% female) with a mean age of 27.5  8.0 years. There was a statistically significant difference in VAS, AOFAS Ankle-Hindfoot Scale, and Karlsson-Peterson scores (p < 0.001) at 6 and 12 months. There is an increase of the AOFAS Ankle-Hindfoot Scale from preoperative (Md = 61.0 (8.0)) to 6 months and 12 months postoperative (Md = 96.0 (6.0)). The Karlsson-Peterson score improved from preoperative (Md = 45.0 (5.0)) to 6 months postoperative (Md = 95.0 (6.0)) and 12 months postoperative (Md = 95.0 (5.0)). Patient satisfaction at 12 months was high (91.7% very satisfied, 8.3% satisfied), with two minor complications. Association analysis revealed no significant impact of age or gender on 12 months postoperative results.

Conclusions

The arthroscopic technique with knotless anchors showed excellent results in short-term functional postoperative recovery. Further studies should be carried out comparing the surgical technique with open Broström-Gould surgery and/or arthroscopic technique with knot systems.
